4、 The occa ad the - “of" s. ite new y-r god h 本方案適用于xxxxxxHI標(biāo)工程項(xiàng)目經(jīng)理部的黃家梁施工區(qū)、水碼門施工區(qū)的1# 2#、3# 4#隧洞以及其他施工。四、編制原則1、堅(jiān)持安全第一、預(yù)防為主、綜合治理的方針2、認(rèn)真、全面、系統(tǒng)閱讀施工合同,深刻領(lǐng)會(huì)和貫徹業(yè)主意圖及業(yè) 主對承包商的各項(xiàng)要求。3、貫徹執(zhí)行各項(xiàng)技術(shù)標(biāo)準(zhǔn)、安全技術(shù)規(guī)程。執(zhí)行業(yè)主對本工程建設(shè)的各項(xiàng)要求,采取現(xiàn)代化管理手段和施工項(xiàng)目管理模式,優(yōu)化資源配置,實(shí)行動(dòng)態(tài)管理,以適應(yīng)施工組織安排的要求。4、對現(xiàn)場安全管理堅(jiān)持實(shí)施全員、全方位、全過程嚴(yán)密監(jiān)控。5、對危險(xiǎn)因素進(jìn)行辨識、在辨

18、e ngthening"four8、施工現(xiàn)場安全生產(chǎn),文明施工,合格率100%;9、沒有社會(huì)相關(guān)方面和員工的重大投訴;10、粉塵、污水、噪聲排放達(dá)到標(biāo)準(zhǔn)。第二章 工程概況一、工程概況1、本標(biāo)段隧洞工程原設(shè)計(jì)由青龍嘴渠首取水工程(底欄柵重力壩)、渡槽(長0.0457km) 、干渠(隧洞:樁號; 0+000-4+450 ) ,另有三條支洞,其中一個(gè)為溢流支洞,其位置為:樁號在干渠的0+0.00 段處為溢流支洞,( 溢流支洞長30.0 m) ; 樁號在干渠的2+400.98 段處 ( 肖家灣支洞 : 支洞長 30.0 m); 樁號在干渠的3+622.0 段處 (黃家梁支洞 : 支洞長:48
19、.0m) 。變更后取消了肖家灣支洞、黃家梁支洞、干渠的 0+0.00 段處為溢流支洞,增加了黃家梁渡槽。1)由于設(shè)計(jì)變更隧洞施工的通風(fēng)排煙、排水、出渣運(yùn)輸、澆筑運(yùn)輸?shù)染l(fā)生變化,減少了施工作業(yè)面,增加了單面施工作業(yè)距離,使供電、 供風(fēng)、 排水、 排煙的難度增大,使料場、 渣場、 庫房的壓力增大,2)原2#洞到肖家支洞約2400ml單頭施工1200ml肖家灣支洞到黃家梁支洞1222m單頭施工611ml3)變更后2#隧洞進(jìn)口到3#隧洞進(jìn)口之間的實(shí)際距離3226m(0+04A3+269),單頭施工1613m。4)隧洞開挖后揭示的地質(zhì)圍巖與原設(shè)計(jì)出入較大,隧洞穿過煤窯老采空區(qū)及煤層,塌方、有毒有害的安
20、全防范措施是本工程的重點(diǎn)。二、工程地質(zhì)區(qū)域地質(zhì):工程區(qū)位于長江北岸,地處四川盆地的東北緣,總體屬川 東平行嶺谷的一部分。位于四川盆地沉降帶(為新華夏系第三沉降帶 之組成部分)之川東褶帶東緣,因受大巴山弧形構(gòu)造阻擋,構(gòu)造線向 東逐漸彎曲,其方向由北北東向逐漸偏轉(zhuǎn)為近東西向。 地質(zhì)構(gòu)造形態(tài) 以褶皺為主,此區(qū)還有巖溶地下水分布。由于受逆斷層構(gòu)造的影響, 地層破碎,節(jié)理裂隙發(fā)育不規(guī)則,圍巖自身穩(wěn)定性差,隧洞開挖后碎 裂間隙滲入隧洞內(nèi)。三、工程水文隧洞區(qū)地下水主要類型為基巖裂隙水。 構(gòu)造裂隙水以及大氣降水,巖 層富水性較弱中等。隧洞進(jìn)口端地形較緩,埋深較淺,雨季時(shí)受地 表水易于補(bǔ)給,雨季時(shí)地下水相對較豐
21、富。通過地質(zhì)調(diào)查和資料的分 析,隧洞洞身段埋藏較深,受構(gòu)造影響巖體節(jié)理裂隙較發(fā)育,地下與 賦水性相對較好,類溪水豐富易出現(xiàn)較大涌水,雨季時(shí)受地表水補(bǔ)給, 故富水量也變化相對大,對隧洞開挖產(chǎn)生不利影響。四、工程危險(xiǎn)源分析1、危險(xiǎn)源的種類(1)、按給人造成傷害的類別進(jìn)行分類,一般有以下幾類:隧洞施工一般可能遇到的不良地段和特殊地段有:滑坡、坍塌、巖堆、偏壓地 層、巖溶、高應(yīng)力高強(qiáng)度地層、松散地層、軟土地段,膨脹地層、軟 弱黃土地層、含水未固結(jié)圍巖、溶洞、斷層、巖爆、流沙、瓦斯溢出f back bone backbone role;t ofullstrengt heni ng members you

37、正常的施工生產(chǎn)等常規(guī)的活動(dòng)和臨時(shí)搶修等非常規(guī)的活動(dòng);三是所有的施工設(shè)備、設(shè)施,也包括建設(shè)、監(jiān)理、檢測等各相關(guān)單位的設(shè)備;四是所有場所和環(huán)境,并充分考慮其在過去、現(xiàn)在、將來和正常、異常、緊急等狀態(tài)。3、危險(xiǎn)評估從人的不安全行為、作業(yè)活動(dòng)的不安全因素、設(shè)備設(shè)施和周圍環(huán)境的不安全狀態(tài)等方面,對隧洞施工可能存在的危害進(jìn)行識別?,F(xiàn)有的預(yù)防措施不能防止事故發(fā)生的,很可能造成人員傷亡的,列為重要危險(xiǎn)源,其他可能造成傷害的判定為一般危險(xiǎn)源。(1) 、 如果現(xiàn)有的防范措施不能防止事故發(fā)生,很可能造成人員死亡的危險(xiǎn)源判定為重大危險(xiǎn)源,其他為一般風(fēng)險(xiǎn)源。把分析判斷的結(jié)果填入危險(xiǎn)源辨識、分析評估表,并按傷害類別匯總,
38、填危險(xiǎn)源清單 。(2) 、對分析評估出的危險(xiǎn)源指定相應(yīng)的控制措施,對于重要的危險(xiǎn)源也可以形成單獨(dú)的安全管理方案,編入工地安全自控標(biāo)準(zhǔn)。(3)、工程施工及方案等發(fā)生變化時(shí),應(yīng)進(jìn)行危險(xiǎn)源和控制措施的再 評審,如有必要,應(yīng)重新制定措施,重新編制危險(xiǎn)源辨識清單和目標(biāo)和管理方案一覽表,并在實(shí)施前,以書面形式或電子郵件方式報(bào)公司安全質(zhì)量部。4、風(fēng)險(xiǎn)控制和管理對評估出的危險(xiǎn)源制定控制措施,有針對性地進(jìn)行安全技術(shù)交底。對 危險(xiǎn)性較大的專項(xiàng)工程應(yīng)當(dāng)編制專項(xiàng)工程施工方案。安全生產(chǎn)管理部負(fù)責(zé)制定重大危險(xiǎn)源識別和控制管理。建立工程項(xiàng)目施工安全重大危險(xiǎn)源的臺帳,加強(qiáng)重大危險(xiǎn)源的監(jiān)控管理。對本工程項(xiàng)目的施工安全重大危險(xiǎn)源
39、應(yīng)予以公示,并在其部位懸掛安全警示標(biāo)志。項(xiàng)目部應(yīng)對重大危險(xiǎn)源實(shí)施動(dòng)態(tài)管理, 項(xiàng)目負(fù)責(zé)人、專職安全管理人 員應(yīng)當(dāng)全面準(zhǔn)確的掌握工程項(xiàng)目的施工安全重大危險(xiǎn)源, 加強(qiáng)對施工 安全重大危險(xiǎn)源的檢查。編制安全應(yīng)急救援預(yù)案,配備應(yīng)急救援人員、器材設(shè)備,定期組織演 練。工區(qū)項(xiàng)目部在開工前必須由負(fù)責(zé)人組織施工技術(shù)、設(shè)備物資、交通消 防和安全管理等方面的人員(必要時(shí)可選聘相關(guān)行業(yè)專家或公司有關(guān) 部門及人員),對所承擔(dān)的工程進(jìn)行全面分析、研究,根據(jù)工區(qū)項(xiàng)目 部指定的安全目標(biāo)和國家、行業(yè)有關(guān)法律、法規(guī)、標(biāo)準(zhǔn),結(jié)合本單位和兄弟單位事故案例、各行業(yè)安全技術(shù)規(guī)則和現(xiàn)有安全管理制度、操 作規(guī)程所防范的危害,以及相關(guān)工程施工

50、,對工程施工中可能存在的危險(xiǎn)源進(jìn)行分類,評估,分級,并采取措施控制。5、危險(xiǎn)源管理制度(1) 、安全技術(shù)交底。工程施工前,根據(jù)已辨識出施工中存在的危險(xiǎn)源,將安全技術(shù)要求納入項(xiàng)目和工點(diǎn)安全技術(shù)交底中。(2) 、防護(hù)措施、設(shè)備、機(jī)具、個(gè)人防護(hù)用品配備。施工前,根據(jù)工地安全標(biāo)準(zhǔn)或安全管理方案,配備相應(yīng)的安全防護(hù)設(shè)施、設(shè)備、 機(jī)具、個(gè)人防護(hù)用品。(3) 、教育和培訓(xùn)。對關(guān)鍵工序的操作、管理和其他相關(guān)人員進(jìn)行相應(yīng)的安全教育和技能培訓(xùn),達(dá)到相應(yīng)的安全能力后,方可上崗。(4) 、檢查監(jiān)控。安全管理人員或安全管理方案規(guī)定的其他負(fù)有監(jiān)控職責(zé)的人員,必須對已開工的工程實(shí)施工地安全標(biāo)準(zhǔn)或安全管理方案的情況和相關(guān)目標(biāo)
51、、指標(biāo)的完成情況進(jìn)行定期檢查,并將情況填入安全日志。對與存在的與工地安全標(biāo)準(zhǔn)和安全管理方案不符合或?qū)δ繕?biāo)、指標(biāo)的實(shí)現(xiàn)有影響的缺陷,應(yīng)及時(shí)向有關(guān)人員指出,并按規(guī)定的時(shí)限向分管領(lǐng)導(dǎo)報(bào)告;對于嚴(yán)重威脅操作人員安全的,有權(quán)要求立即停工。(5) 、糾正和預(yù)防。對于施工中出現(xiàn)的事故、未遂事故、檢查中發(fā)現(xiàn)的缺陷和上級指出的缺陷,應(yīng)及時(shí)組織有關(guān)管理人員,必要時(shí)請上級相關(guān)部門人員及相關(guān)行業(yè)專家進(jìn)行調(diào)查分析,擬定并評審糾正和預(yù)防措施, 并對專門的安全管理方案進(jìn)行必要的變更;糾正和預(yù)防措施實(shí)施后,要評審其是否達(dá)到預(yù)期效果;糾正和預(yù)防措施完成后,應(yīng)作專門的記錄。對應(yīng)上級指出的缺陷,要將實(shí)施情況向上級有關(guān)部門專題 報(bào)告
52、。(6)、根據(jù)制定的控制措施,確定工區(qū)項(xiàng)目部各部室或人員的職責(zé),編入工地安全自控標(biāo)準(zhǔn);并結(jié)合本工程項(xiàng)目情況對安全目標(biāo)、 指標(biāo)進(jìn) 行分解,對重大危險(xiǎn)源編制目標(biāo)和管理方案一覽表(附表三)。并在開工前,將危險(xiǎn)源清單和目標(biāo)和管理方案一覽表以書面形 式或電子郵件方式報(bào)廈深局指和公司安全質(zhì)量部。6、事故應(yīng)急救援(1)、工區(qū)項(xiàng)目部制定、評審安全事故應(yīng)急預(yù)案。(2)、工區(qū)項(xiàng)目部應(yīng)急預(yù)案體系由綜合應(yīng)急預(yù)案、與風(fēng)險(xiǎn)類別相對應(yīng) 的專項(xiàng)應(yīng)急預(yù)案和關(guān)鍵工作崗位的現(xiàn)場處置方案等三類方案構(gòu)成。(3)、應(yīng)急預(yù)案的編制,按照國家安檢總局發(fā)布的生產(chǎn)單位安全生 產(chǎn)事故應(yīng)急預(yù)案編制導(dǎo)則(以下簡稱編制導(dǎo)則)的規(guī)定進(jìn)行編制。(4)、結(jié)合
