肺炎克雷伯菌相關(guān)血流感染臨床特征分析摘要:本研究旨在探討肺炎克雷伯菌相關(guān)血流感染的臨床特征,以提高對該病的識別和治療。收集了2015年至2019年本院收治的32例肺炎克雷伯菌血流感染患者資料,對患者的臨床特征、病原學(xué)檢測結(jié)果、治療情況、轉(zhuǎn)歸等進(jìn)行回顧性分析。結(jié)果顯示:患者多為老年男性,多有基礎(chǔ)疾病,其中呼吸系統(tǒng)疾病為最常見,全身炎癥反應(yīng)明顯,白細(xì)胞計(jì)數(shù)和C反應(yīng)蛋白升高,多數(shù)合并有其他感染。所有患者均從呼吸系統(tǒng)感染開始,其中絕大部分合并有肺炎,常出現(xiàn)相應(yīng)癥狀和體征,如發(fā)熱、咳嗽、呼吸困難、肺部體征陽性等。藥敏試驗(yàn)顯示肺炎克雷伯菌對多種抗生素存在不同程度的耐藥性。采用抗感染及營養(yǎng)支持治療,部分患者進(jìn)展迅速,但亦有部分患者病情惡化,最終死亡。綜上所述,肺炎克雷伯菌相關(guān)血流感染對病情影響嚴(yán)重,診斷及早、治療及時(shí)至關(guān)重要。

Abstract:ThisstudyaimedtoexploretheclinicalfeaturesofKlebsiellapneumoniae-relatedbloodstreaminfectionstoimproverecognitionandtreatmentofthisdisease.Theclinicalcharacteristics,pathogendetectionresults,treatment,andoutcomesof32patientswithK.pneumoniaebloodstreaminfectionsadmittedtoourhospitalfrom2015to2019wereretrospectivelyanalyzed.Theresultsshowedthatmostpatientswereelderlymenwithunderlyingdiseases,ofwhichrespiratorysystemdiseaseswerethemostcommon.ThesystemicinflammatoryresponsewassignificantwithelevatedwhitebloodcellcountsandC-reactiveproteinlevels,andmostpatientshadotherconcurrentinfections.Allpatientsbeganwithrespiratorysysteminfections,andthemajorityofthemhadpneumoniawithcorrespondingsymptomsandsignssuchasfever,cough,dyspnea,andpositivepulmonarysigns.DrugsensitivitytestsdemonstratedvaryingdegreesofresistanceofK.pneumoniaetomultipleantibiotics.Antibiotictreatmentandnutritionalsupportwereused,andsomepatientsprogressedrapidly,butothersworsenedandeventuallydied.Inconclusion,K.pneumoniae-relatedbloodstreaminfectionshaveasevereimpactonpatientprognosis,andearlydiagnosisandtimelytreatmentarecrucial.

OnepromisingareaofresearchisthedevelopmentofvaccinesagainstK.pneumoniae.Therearecurrentlynovaccinesforthisbacteria,butseveralvaccinecandidateshaveshownpromiseinpreclinicalstudies.Thesevaccinesworkbyinducinganimmuneresponseagainstcertainantigensonthesurfaceofthebacteriathatareessentialforitssurvivalandvirulence.Bytargetingtheseantigens,thevaccinecanpreventthebacteriafromcolonizingthehostandcausinginfection.
