BSERVATION&ACTIONTAKEN:

Observation:Unsafecraneoperationpracticesandequipmentdeficiencies,posingasignificantriskofaccidents,injuries,andequipmentdamage.

1.Boththecrawlercraneandthetruck-mountedmobilecranelackaSafeWorkload(SWL)indicator.

2.Thetruck-mountedmobilecraneisdesignedforatwo-partline(meaningitisratedtoliftacertainloadusingadoublelineofcable),whiletheriggingblockisdesignedforathree-partline(meaningitisdesignedtoliftacertainloadusingatriplelineofcable).

3.Thismismatchinlinedesign-Mobilecranecouldbeoverloadediftheoperatorassumesthethree-partblockhasthesameliftingcapacityasthetwo-partline,leadingtopotentialstructuralfailureandaccidents.

4.Thetruck-mountedmobilecranehasnoLoadLimitIndicator.ALoadLimitIndicator(LLI)isdesignedtoprovideanaudibleorvisualwarningtotheoperatorwhenthecraneapproachesitsmaximumloadcapacity.

5.Thewireropeclippositionbetweentheliveendanddeadendisunsafelyconnected,withthetailendnotdirectlyclampingdirectlytothewedgeorliveendrope.Whentheclipisincorrectlyplaced,thewireropecansliporcomeloose,potentiallycausingropefailure&LiftingMechanismfailure.
