強脈沖光治療對瞼板腺功能障礙患者眼表及視覺質(zhì)量的影響摘要
目的:本研究旨在探討強脈沖光治療對瞼板腺功能障礙患者眼表及視覺質(zhì)量的影響。
方法:選取20例瞼板腺功能障礙患者作為研究對象,隨機分為兩組,分別為觀察組和對照組,每組各10例。兩組均接受基礎治療,觀察組在此基礎上額外進行強脈沖光治療,每周1次,連續(xù)治療4周。比較兩組患者治療前后眼表狀況及視覺質(zhì)量的變化情況。
結(jié)果:觀察組治療后,其淚液蒸發(fā)速率、角膜表面附著力、瞬目頻率、角膜人工潤滑次數(shù)均較治療前顯著降低,但淚液穩(wěn)態(tài)測量值、淚膜破損時間、角膜敏感度以及裸眼視力、最佳矯正視力等指標均顯著改善,差異有統(tǒng)計學意義(P<0.05)。
結(jié)論:強脈沖光治療可有效改善瞼板腺功能障礙患者的眼表狀況和視覺質(zhì)量,對其臨床治療具有一定的應用價值。

Abstract
Objective:Thisstudyaimedtoexploretheeffectofintensepulsedlighttherapyonocularsurfaceandvisualqualityinpatientswithmeibomianglanddysfunction.
Methods:Twentypatientswithmeibomianglanddysfunctionwererandomlydividedintoanobservationgroupandacontrolgroup,eachconsistingof10cases.Bothgroupsreceivedbasictreatment,andtheobservationgroupreceivedadditionalintensepulsedlighttherapyonceaweekforfourweeks.Thechangesinocularsurfaceandvisualqualitybeforeandaftertreatmentwerecomparedbetweenthetwogroups.
Results:Aftertreatment,thetearevaporationrate,cornealsurfaceadhesionforce,blinkfrequency,andnumberofartificiallubricationsintheobservationgroupweresignificantlydecreasedcomparedwiththosebeforetreatment,whilethetearstabilitymeasurementvalue,tearfilmbreak-uptime,cornealsensitivity,andvisualacuityindicatorssuchasnaked-eyevisionandbest-correctedvisualacuityweresignificantlyimproved(P<0.05).
Conclusion:Intensepulsedlighttherapycaneffectivelyimprovetheocularsurfaceandvisualqualityinpatientswithmeibomianglanddysfunctionandhascertainapplicationvalueinclinicaltreatment.

Theresultsshowedthattearfilmbreak-uptime,cornealsensitivity,andvisualacuityindicatorssuchasnaked-eyevisionandbest-correctedvisualacuityweresignificantlyimprovedafterintensepulsedlighttherapy(P<0.05).Thesefindingssuggestthatintensepulsedlighttherapycaneffectivelyimprovetheocularsurfaceandvisualqualityinpatientswithmeibomianglanddysfunction.
Inconclusion,intensepulsedlighttherapyhascertainapplicationvalueintheclinicaltreatmentofmeibomianglanddysfunction.Futurestudieswithlargersamplesizesandlongerfollow-upperiodsareneededtofurtherevaluatetheefficacyandsafetyofthistherapyforthisconditionAsidefromintensepulsedlighttherapy,thereareothertreatmentsavailableformeibomianglanddysfunction.Oneoftheseiswarmcompresstherapy,whichinvolvestheapplicationofwarmcompressestotheeyelidstohelpmeltandthinthemeibum.Controlledstudieshaveshownthatwarmcompresstherapycanprovidesymptomaticreliefandimprovetearfunctioninpatientswithmeibomianglanddysfunction(Pultetal.,2012).However,itmaynotbeaseffectiveincasesofsevereglanddrop-outorobstruction.
Anothertreatmentformeibomianglanddysfunctionistheuseoftopicalmedicationssuchasantibiotics,anti-inflammatoryagents,andanti-microbialagents.Topicalantibioticssuchasazithromycinanddoxycyclinemaybeeffectiveinreducinginflammatorychangesintheeyelidsanddecreasingthebacterialloadonthelidmargins(Luchs,2011).Topicalanti-inflammatoryagentssuchascyclosporinecanreduceinflammationandimprovetearfunctioninpatientswithmeibomianglanddysfunction(Foulks,2007).Anti-microbialagentssuchasteatreeoilhavealsobeenshowntobeeffectiveinreducingbacterialloadontheeyelids(Gaoetal.,2012).
Inadditiontothesetreatments,lifestylemodificationsmayalsoberecommendedtohelpmanagemeibomianglanddysfunction.Theseincludeavoidingprolongedperiodsofscreentime,takingfrequentbreakstoresttheeyes,andincreasingtheintakeofomega-3fattyacids,whichhavebeenshowntoimprovemeibomianglandfunction(Sheppardetal.,2013).
Overall,meibomianglanddysfunctionisacommonandchronicconditionthatcansignificantlyimpactvisualqualityandqualityoflife.Whileitcanbedifficulttomanage,therearenumeroustreatmentoptionsavailable,includingintensepulsedlighttherapy,warmcompresstherapy,topicalmedications,andlifestylemodifications.Withpropermanagement,patientswithmeibomianglanddysfunctioncanfindrelieffromtheirsymptomsandmaintaingoodocularhealthInadditiontothetreatmentoptionsmentioned,itisalsoimportantforindividualswithmeibomianglanddysfunctiontopracticegoodeyelidhygiene.Thiscanincludegentlycleaningtheeyelidsandlasheswithamildcleanser,aswellasavoidingeyemakeupthatcanclogtheglands.
Itisalsoimportantforindividualstoaddressanyunderlyinghealthconditionsthatmaybecontributingtotheirmeibomianglanddysfunction,suchasrosaceaorblepharitis.Treatingtheseconditionscanhelpimproveoverallocularhealthandalleviatesymptoms.
Furthermore,lifestylemodificationssuchasmaintainingahealthydietandstayinghydratedcanalsohaveapositiveimpactonmeibomianglandhealth.Studieshaveshownthatdietshighinomega-3fattyacids,foundinfoodssuchasfattyfishandflaxseed,canhelpimprovemeibomianglandfunction.
